On iPhone open the shortcuts app tap + select Add Action. Go to apps and select Chrome. Select Open Urls in Chrome. Enter the site URL you want tap top right. Select Add to Home Screen. Enter the app name. Press Add. On Android open Chrome tap three dots on the top right. Select Add to Home Screen. Press Add.
